has EXTENDED the deadline for submission of nominations
for the UP Medical Alumni Society (UPMAS) ALUMNI AWARDS to
SEPTEMBER 30, 2015.
Attached, in one document: Guidelines; Criteria for the different award categories; Nomination form(s) for you to fill up.
Please include a short write-up (about 250-300 words) summarizing why the nominee deserves to receive the award
he/she is being nominated for.
Kindly submit accomplished form(s) to
