Associate Prof Lee Kheng Hock, Deputy CEO of SingHealth Community Hospitals and Director of the World Health Organization Collaborating Centre for Social Prescribing delivered an informative talk on how social prescribing can improve patient well-being and social integration.
The World Health Organization defines social prescribing as a means for health-care workers to connect patients to a range of nonclinical services in the community to improve health and well-being. It helps address the underlying causes of patients’ health and well-being issues, and presents a more holistic approach to health care as it promotes community-based integrated care.
“The transition from hospital to community-based care is a critical juncture in the healthcare system, where patients’ well-being can be greatly impacted,” said Prof. Lee during the discussion on Nov. 3, 2025, at the College of Nursing auditorium. He also presented a social prescription model, which integrates community-based services and resources into the care continuum that addresses the multi-faceted needs of patients.
The topic was explored further with ideas from Dr. Leilanie A. Nicodemus, from the College of Medicine and Director of the Newborn Screening Reference Center, and Dr. Katherine Ann V. Reyes, Program Lead of the Health Promotions Program at the National Institutes of Health. The discussion was moderated by Dr. Louricha A. Opina-Tan, Associate Professor at the Department of Family and Community Medicine, UP College of Medicine.
