2. Endorsement letter signed by the Thesis/Research Adviser, Department Chair, and College Dean
3. Thesis panel approval certificate
4. Ethics review approval/exemption certificate
5. RGAO registration certificate (register your project at http://rgao.upm.edu.ph/registration)
