We would like to announce the short course offerings for 2009. These include the following:

| Human Resource for Health Management | June 1-5, 2009 |
A file found below in pdf format is attached for a more detailed description of each course. May we kindly request for this information be disseminated in your hospital.
The fee is P10,000 for each training course except for ICD -10 and Hospital Information System. Fees are inclusive of training kit, morning and afternoon snacks, batch directory, batch picture, and compilation of lectures in CD. The course fee excludes hotel accommodation and lunch. Discounts are as follows:
Ø 10% discount for full payments one week before the said schedule
Ø 15% discount for full payments two weeks before the said schedule
The fee for ICD -10 and Hospital Information System is P8,000 for each training course with the same arrangement as above but has no discount!
At the back is a Reply Slip/Reservation Form, which we request you to accomplish and return to us soonest to ensure yourself of a slot in the course. A reservation fee of P500.00 is non-refundable but will be deducted from the full cost of the course fee (in case the registrant fail to attend the course). Payments may be in cash or cheque payable to the UP-CPH Department of Health Policy and Administration. Or deposit to any BPI branch – Savings account name: UP-CPH Department of Health Policy and Administration; account #: 3063-5296-04.
